Q for Andrew: when you looked at JP’s tongue in our case history for this month (Therapeutics class), you asked where she was in her cycle when you saw the sublingual blood stagnation. Could you please explain this connection? Does sublingual stagnation build up before the cycle starts and decrease after? Or are you looking to see if the stagnation is there when it shouldn’t be (i.e., if she’s in the follicular phase?) Or something else?

Andrew said: You got it! TCM says that the sublingual veins will typically become more engorged/darker starting 7-10 days before day 1 of the cycle. They should be at their thinnest/lightest by day 5-7 when bleeding has stopped. This follows the normal physiological cycle of blood stagnation premenstrually and relative blood deficiency after bleeding. If the person does not fit this expected physiological pattern, it is an indication that there is some imbalance that needs to be addressed. What is the imbalance? We would need to analyze the entire case to figure out the pattern.
