Hyperemesis

Have you ever worked with someone who was experiencing hyperemesis? In this case, this mother is in her 2nd pregnancy & is experiencing severe nausea, can't keep food down. Her first pregnancy was the same. What would be a TCM and/or energetic framework to understand this pattern? To me this seems a condition extreme enough in nature that herbs might not need to be tailored necessarily but that there might be a few key herbs for which "extreme nausea"/"can't keep food down" are black letter symptoms, and are also herbs safe for pregnancy. Would that be the case? Thank you!

Yes, I have and it is very tricky, Ginger is safe and sometimes works (it also can make it worse for some women), Ume plums help some women (you suck on the pickled plum), from there becomes more problematic herbs such as Ban Xia (processed Pinellia) can work, but their safety during pregnancy is rather questionable. It is always used in TCM formulas never as a simple. I know some women who used Cannabis, but the safety of that during pregnancy is also highly questionable. Some studies suggest B1 and B6 may offer some relief (I have not seen much benefit when I gave it to women with Hyperemesis gravidarum). Acupuncture has helped some women and so has hypnosis. The problem is all of treatments have helped some women and not others, nothing has consistently helped most of them.
